Frequently Asked Questions

Important Information for 2025 Renewals

  • All licenses must be renewed by December 31, 2025, regardless of the date of licensure.
  • Renewal FAQs here: https://cca.hawaii.gov/pvl/news-releases/dental_announcements/
    100% online/virtual CEs are accepted, except for the hands-skill portion of BLS, which must be taken in-person.
  • Inactive status licenses must be renewed by the renewal deadline to remain on inactive status.
  • Dentists with the special privilege permit to administer anesthesia must submit HARD COPY renewals only.
  • Contact the Board by emailing dental@dcca.hawaii.gov for any questions.

If requesting a waiver for the CE requirement:

  • If you are requesting a CE waiver or extension pursuant to HAR §16-79-147, you must submit your request in writing with a HARD COPY only renewal form, which will be pending approval by the Board.
  • If you submit your renewal form without meeting the CE requirements and without having an approved CE waiver/extension, you are falsely attesting, and your license may be subject to disciplinary action.

Community Service Dental Hygienist Renewal Information (Renew every year within 1 month to license expiration date)

  • All Licenses regardless of issuance date, are subject to renewal within one month prior to the license expiration date.
IT IS THE RESPONSIBILITY OF THE LICENSEE TO RENEW BY LICENSE EXPIRATION DATE ON A YEAR TO YEAR BASIS. (no further notice is provided)
Community Service Dental Hygienist Renewal Application Form

Licenses not renewed by license expiration date are unlicensed and may not practice and must reapply as a new applicant and meet current licensing requirements.

On-time renewals for the licensing period of January 1, 2024 to December 31, 2025 (DT w/Anesthesia Privilege must renew by hardcopy only and submit facility inspection form)

Regain Active Status

Restoration Information (Restore within 2 years after license expiration)

(DT, DH) Forfeited licenses may be restored within 2 years and subject to meeting restoration requirements. Failure to restore a forfeited license within 2 years after the license expiration date will result in the termination of the license and the person must apply as a new applicant and meet current licensing requirements.
