How to Request: Address/Name Change/Pocket ID/Wall Certificate

Print Your License Pocket Id and Wall Certificate Online

Effective April 18, 2019, you must retrieve and print your pocket ID card and wall certificate via your MyPVL account.  No other notice will be provided.

Address Changes Accepted Online, Beginning November 4, 2019

List of Licenses that may submit online address changes
(License Status MUST be “Current, Valid & In Good Standing”)
Processing time for online address changes: 3-5 business days

Hardcopy Address/Name Change Forms

  • Submit address/name change form only if you cannot submit address changes online;
  • Submit address/name change form if you have a name change;
  • Select address change form applicable for your license type and follow instructions contained in form;
  • Incomplete requests will not be processed;
  • Your records will not be updated if your license is not current, valid and in good standing;
Processing time for hardcopy address/name change requests:

20-30 business days from receipt of request form
HEALTH CARE PROFESSIONALS – Address/Name Change Request Form
(Physician, osteopathic Physician, Physician Assistant, Podiatrist, Emergency Medical Technicians, Naturopathic Physician and Nurse)
REAL ESTATE PROFESSIONALS – Address/Name Change Request Form
(Real Estate Broker, Real Estate Salesperson, Condominium Management Association, Condominium Hotel Operator)
ALL OTHER LICENSE TYPES – Address/Name Change Request Form
